This event is for School Age Children ONLYL Grades K-12
Completion of a medical screening & all informed consent documents is required for all persons registering for the Flu Vaccination Clinic
The medical screening has been developed to determine if you are eligible to receive the Flu Vaccination
Each person (adult and/or child) registering for a flu vaccination appointment is required to complete the Texas Department of State Health Services Immunization Registry Consent
